Job Description:
A company is looking for a Care Manager, LTC fluent in Mandarin,
Cantonese, or Fujianese.
Key Responsibilities
Advocates and educates members on services and self-management
techniques
Conducts assessments and develops care plans aligned with treatment
goals
Coordinates with healthcare providers and community resources to
ensure effective care
Required Qualifications
NYS RN or LCSW/LMSW (any state) for Medical Care Management
Fluency in English and one of the following: Mandarin, Cantonese,
or Fujianese
Experience managing member information in a shared network
environment
Knowledge of community health practices for frail adults and
cognitively impaired seniors
Experience as a Care Manager is preferred
