Job Description:
A company is looking for an RN Care Manager to oversee patient
care plans and collaborate with field staff.
Key Responsibilities
Review and revise the Plan of Care (POC) to ensure accuracy and
compliance with regulatory standards
Participate in interdisciplinary team meetings and manage
healthcare costs through effective care decisions
Assess patient clinical, functional, and behavioral health status
for timely follow-up and service allocation
Required Qualifications
Registered Nurse licensure required
Two years of home care field experience preferred
One year of clinical review preferred
Two years of case management and/or utilization management
experience preferred
Knowledge of Medicare, Medicaid, and compliance guidelines for
patient clinical care
