• Employment Type: Travel Registered Nurse - Home Care West Philadelphia-19143 13 weeks Description: We may need weekend coverage with this contract, would be 1 weekend every 4 weeks, would provide shift days during the week if needed. Job Summary: We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality, patient-centered care in a home care setting. The Home Care RN will assess patients' conditions, develop care plans, administer treatments, and educate patients and families on disease management and home healthcare best practices. Key Responsibilities:
  • Patient Assessment & Care Planning:
    • Conduct initial and ongoing assessments of patients' medical conditions.
    • Develop and update individualized care plans in collaboration with healthcare teams.
    • Clinical Care & Treatment:
      • Administer medications, wound care, and other prescribed treatments.
      • Monitor and document patients' progress and response to treatments.
      • Provide post-operative care, chronic disease management, and rehabilitation support.
      • Patient & Family Education:
        • Teach patients and families about medications, disease management, and home care techniques.
        • Offer guidance on maintaining a safe home environment.
        • Coordination & Communication:
          • Work closely with physicians, therapists, social workers, and other healthcare professionals.
          • Communicate changes in patients' conditions to the healthcare team.
          • Compliance & Documentation:
            • Maintain accurate and timely medical records in compliance with regulations.
            • Follow all healthcare policies, infection control protocols, and safety guidelines. Qualifications:
              • Education & Licensure:
                • Associate or Bachelor's degree in Nursing (ADN/BSN).
                • Active Registered Nurse (RN) license in the state of practice.
                • BLS/CPR certification (required).
                • Experience:
                  • Minimum 1-2 years of nursing experience (home care, hospice, or acute care preferred).
                  • Skills & Competencies:
                    • Strong assessment and critical-thinking skills.
                    • Excellent communication and patient interaction abilities.
                    • Ability to work independently and manage multiple cases.
