Job Description:
A company is looking for a Transition Specialist to manage the
coordination of member transitions in the community.
Key Responsibilities
Support care coordination teams and providers in developing
effective transition plans for members
Assist with the transition of members based on enrollment or care
service changes
Track transition metrics and ensure proper documentation of care
transitions
Required Qualifications
Bachelor's degree required
2 - 4 years of related experience or equivalent experience
Experience in care coordination or health services preferred
Knowledge of community health resources and services
Familiarity with transition planning and care management
processes
